The ten educational modules are designed around the hospital-driven quality improvement approach for implementing these strategies presented in the Million Hearts®/AACVPR Cardiac Rehabilitation Change Package. The TAKEheart initiative training curriculum seeks to help hospitals implement quality-improvement efforts to increase patient participation in CR. Each module includes an hour-long, expert-led webinar presentation with a discussion. PPT slides and a companion Implementation Guide are provided to reinforce the content. Additional relevant materials from the Million Hearts®/AACVPR Cardiac Rehabilitation Change Package and other sources are available on the specific module page.

Each module specifies an intended audience, which is identified on the individual module page. However, this is made as a suggestion rather than a limitation. All are welcome to watch and review each module.

TAKEheart Partner Hospitals will receive email with a link to register. Others may register on the individual module pages on the TAKEheart website.

Yes, the training curriculum is accredited through the American Board of Quality Assurance and Utilization Review Physicians (ABQAURP)*. Each module is 1.0 AMA PRA Category 1 credit. CEUs will only be granted for individuals who attend the live training.

Yes, education credits are limited to physicians and nurses on the Partner Hospital multidisciplinary teams. CEUs will only be granted for individuals who attend the live training.

No. The TAKEheart training curriculum is available for any hospital that wishes to increase patient participation in cardiac rehabilitation.

Each module is independent and can be accessed separately at any time after it has been posted on the TAKEheart website. However, the modules are designed to work together to provide a full quality improvement curriculum. TAKEheart Partner Hospitals are expected to ensure that at least one person from their CR quality improvement team attends each training webinar.

Though each module is independent, hospitals are strongly encouraged to review them in the order in which they are offered. The TAKEheart curriculum mirrors the steps needed to implement automatic referral with care coordination. Individual registration is completed on a rolling basis, but it is recommended that the modules be completed in the order presented on the site.

A new training module is offered each month. However, past modules are available to watch and review at any time.

All webinars are recorded and are made available, along with any accompanying materials, to watch and review at any time. An expectation of Partner Hospitals is that at least one person from the team will attend each training webinar.

Individuals will be able to provide feedback during and after each module through survey questions. Partner Hospitals will also be given the opportunity to provide feedback during group calls. The TAKEheart staff will use the feedback to improve and modify the training content.

Yes, the TAKEheart staff encourages feedback on all aspects of the training curriculum. We may not be able to accommodate all suggestions, but we welcome input. Please email with recommendations.
